The Power of Information – Connected Information for Integrated Care

This section of the document ‘The Power of Information’ looks at integrating information across health and social care, factoring in security and privacy issues, anonymising data and utilising this for research purposes. There is a reference to the Common Assessment Framework Programme for Adults. Examples of integration include e-consultations for chronic kidney disease at Bradford Teaching Hospital, StrokeNav, a barcode medication system at Bristol care homes, integrated healthcare in Worcestershire health economy, clinical portal at Basingstoke and North Hants NHS Trust, VistA, cancer registries and the National Cancer Intelligence Network.

A table of priority areas includes

  • Medicines management
  • Test results exchanged safely across settings
  • Letters being sent electronically between professionals
  • Systems for easy viewing of records
  • Electronic care plans for health and care records
  • Online professional access to shared care records

Context

The 2012 ‘The Power of Information‘ paper by the Department of Health is mentioned in the ‘Personalised Health and Social Care 2020’ paper. This paper has the following headings

  5. Making it happen

The NIB Framework consists of the following principles which are expanded upon in the Personalised Health and Social Care 2020 paper.
